Nuvo Health Medical Centre – Five Dock is a Family Medical Practice open 7 days a week (including Saturday and Sunday). We have Australian trained General Practitioners (GP doctors), nurses, pathology, psychologist and dietitian. We are a private billing practice however discounted rates are available for patients with Pension/Concession card and for children under 16 years of age during weekdays.
Located at 88-90 Great North Rd, Five Dock (previous ANZ bank site, near NAB and St George Bank), our practice is newly built and offers state-of-the-art facilities and equipment.
We accept walk-in patients and appointments over the phone, online via this website or HotDocs app. We serve all patients from Five Dock, Haberfield, Russell Lea, Canada Bay, Ashfield, Concord, Burwood and beyond Sydney. Her special interests are: 1.Women’s Health. This includes CST. 2. Acupuncture. 3. Preventative Health. Including the use of Medicare initiatives . 4. Complementary Medicine.
Dr. Lim's interests are in Women’s Health, Child and Adolescent Health, Mental Health, Preventative Medicine, as well as simple surgical procedures that can be done in the GP setting.
Dr. Lal enjoys all aspects of General Practice - including chronic disease, mental health, children's health and looks forward to helping patients solve their daily medical problems.

WEEKDAY CONSULTATION
Billing Changes – From 4th of September 2023 we are unable to offer bulk-billing for children under 16 or pension/healthcare card holders, however discount rates are available on weekdays. Please check with reception for details.
Out of pocket expenses depend on length and type of consult, we are able to process Medicare rebates at the time of paying in order to minimise your out of pocket expenses.
Walk-in appointments are available however wait times may vary.
Patients without a Medicare Card can pay privately, and claim rebates back from private health fund or travel insurance where appropriate.
WEEKEND CONSULTATIONS
We are a fully private billing practice on weekends and public holidays.
Please note that consultations on Public Holidays attract a higher fee to the usual schedule as these are considered after-hours consultations.
NON-ATTENDANCE POLICY
Please note a Non-Attendance Fee is payable if a patient does not attend their doctors appointment or if the appointment is cancelled with less than two hours notice. The payable fee is $50 for Weekdays or $60 for Weekends and Public Holidays.
If a patient fails to give 48 hours notice or does not attend an allied health appointment, a Non-Attendance Fee of 50% of the consult fee will need to be paid.
VACCINES AND PROCEDURES
For all vaccination and procedural costs please ask one of our friendly receptionists. Your doctor will explain any additional costs to you and obtain your consent prior to receiving treatment.
Any consultations requiring additional services may be charged a procedural equipment fee.
Travel vaccines will incur a private fee also for the purchase of the vaccine as they are not covered by Medicare.

New Patient Information
If this is your first visit to the practice, please bring the following to your first consultation:
A valid Medicare card if you have one (check expiry date) and a Photo ID (eg driver’s licence/student card)
Please arrive 10 minutes ahead of your scheduled appointment time to fill in necessary paperwork including a New Patient Registration Form (Can be downloaded and pre-filled by clicking here: New Patient Registration Form)
If you are a Workcover patient, you will need to inform your Employer prior to the consultation and bring your employer’s contact details for verification to arrange payment through Workcover
If you are coming for a consultation with private billing, or on a Public Holiday or you are a Private patient (without a Valid Medicare card) or you intend on having a procedure eg Dressings/Suturing/Vaccinations, please bring a form of payment. We accept Cash, Eftpos and Credit Cards. We do not accept Cheques.
If you need to change your appointment time please inform us as soon as possible to avoid late cancellation fees.
A council carpark is positioned just behind the medical centre. 2 hour free parking is available here.
Entry is via Waterview street at the rear of the property parallel to Great North Road. We are located just outside one of Five Dock’s most accessible bus stops.

What is your after hours contact information?
If you are an existing patient of the practice and require after hours medical assistance of a non-urgent nature between 7pm and 7am, please call 0491 837 158.
A private non rebatable fee of $200 applies to all after hours telehealth consultations. Home visits will be at the doctors discretion and may incur additional fees. A staff member will contact you on the next business day to process payment.

If I don't have Medicare can I still see a doctor?
Yes, however consults without a Medicare card will attract a private fee. Please refer to our fee schedule or contact one of our receptionists for further details.
Do I need to make an appointment to be seen by a doctor?
We accept walk-in and booked appointments. We recommend booking at your preferred time as we cannot guarantee wait times for patients who come in without an appointment. Our doctors aim to see all patients in a timely manner.
Do I need a long consult?
Our appointments are booked in a standard slot of 15 minutes which usually allows discussion of one matter. Long consultations can be booked upon the doctor’s recommendation. Please advise our receptionists if you would like an extended appointment.
Can I have a phone consult?
Our doctors do offer phone consultations. Where possible, we encourage all patients to attend face-to-face to ensure a detailed clinical assessment can be completed.
How do I get my test results?
Test Results must be given by a doctor at the clinic. Reception staff and nursing staff cannot provide this information to patients. Phone Consultations can be booked with the doctor, but we encourage a face-to-face appointment where possible.
What if I need to see a doctor after hours?
If you require after hours medical assistance of a non-urgent nature, please call 0491 837 158. A private fee of $200 applies to all after hours consultations.
For any urgent medical care please call 000.
Do you send out appointment reminders?
Patients will receive a reminder for their GP appointment 24 hours prior to the appointment. Where possible, please confirm the appointment by replying with a ‘Y’.
For allied health appointments, you will receive a reminder 48 hours before your appointment.

Why do I need to see the nurse?
The nurse assists the doctor to ensure you receive the highest standard of care. The nurse may assist in procedures and take some observations before your consult so the doctor has a more complete picture of your health.
Why is my doctor running late?
Our doctors and receptionists are not able to preempt how long an appointment will take. Seemingly simple problems may need further exploration and discussion. Our doctors are thorough and will not ask a patient to leave until they are confident the highest standard of care has been given. This may cause a delay. Occasionally an unforeseen emergency patient may require medical attention and need to be fitted in. We ask for your understanding in these incidences.
Can I get my medical certificate/referral changed or backdated?
If you would like your medical certificate extended, you will need to return to your GP for an appointment so they can evaluate your condition.
We are not able to backdate medical certificates past the day you saw the doctor for the condition.
Can I get my records transferred to/from your practice?
Yes, if you would like your records sent to us please ask reception for a ‘transfer of record form’ which we will send to your previous practice.
If you would like your records sent elsewhere, please send us your signed consent form and new practice details. Your new practice will have a template for you to fill in.
Do I need an appointment with pathology?
If you have a pathology request form from your doctor, you do not need to make an appointment. Our pathology services are offered on an as-you-arrive basis. However, if you require a glucose tolerance test (GTT or OGTT) please contact reception to book in.
Do I need to fast for my blood test?
Your doctor will give you instructions for your blood test during your consult. If you are unsure, please contact reception and we can transfer you to our pathology collector.
Do you have x-ray facilities?
We do not have imaging on site, the nearest facility is I-Med Five Dock.
